As part of the application process, Children Academy may investigate matters such as an applicant's credit standing, credit
capacity, character, general reputation, and personal characteristics. A criminal background check may be obtained by the
Company through federal, state, and/or local law enforcement agencies. Further information may be obtained through
"consumer reports" prepared by a consumer reporting agency or "investigative consumer reports", which are based on
personal interviews with an applicant's neighbors, friends, associates, or other acquaintances. Upon request, applicants will
be informed whether a consumer report or investigative consumer report has been requested, the name and address of
the reporting agency, and, for investigative consumer reports, additional information regarding the nature and results of
the investigation. If an applicant is denied employment based on information in an investigative consumer report, the Company will so advise the applicant and supply the name and address of the reporting agency making the report.
Subsequent consumer reports may be requested or utilized, during employment to evaluate suitability for continued employment, reassignment, promotion, transfer or other terms and conditions of employment. If any adverse action is taken because of information contained on a consumer report, the Company will advise the employee of the reason for its adverse action and supply the name and address of the reporting agency making the report.
By signing this application, I: 1) authorize the Company to contact all of my employment references, and to investigate and
obtain copies of any records from former employers, educational institutions, other relevant third parties, and any
government or law enforcement agency; and 2) agree to release to the Company and all affiliate entities, their employees,
as well as any person or institution that provides the Company with any information about me, from any and all liability
whatsoever resulting from any such investigation or communication, so long as such information is relevant to my employment. Provide proof of income, including a copy of a recent pay stub or W-2. The Company herein states that it
shall not use any such information for any improper or unlawful purpose.
If required by the company, I hereby consent to having a physical and/or medical examination(s) and/or test(s) conducted
by a physician or other professional of the Company’s choice and understand that an offer of employment may be conditioned upon the results of this examination(s) or test(s). I also agree that should I be employed; I may be required to
take a company directed physical examination as required by state or local childcare laws/regulations. I consent to having
the results of such required physical examinations released to the Company.
All new hires must be fingerprinted and undergo a thorough criminal history screening (CHRI) as well as a thorough child
abuse history screening (CARI). New Hires may start orientation but may not be scheduled to work directly with children until the result of their background check (CHRI) is received and they are cleared to work. Offer of employment is immediately rescinded if it is discovered that an employee falsified or misrepresented their work history, provided false information, or failed to provide pertinent information whether verbally or in writing during their pre-employment screening process. Offer of employment is also rescinded immediately following hit on their child abuse history screening or failed to receive full clearance from the state of New Jersey.
New employees who have been cleared for criminal history may begin working in classrooms but should never allow themselves to be left alone without the oversight of staff who has been cleared. You will be informed when your clearance has been received from the state after which you may be allowed to work alone with children. You will be
terminated if you violate this policy.
